Health Safety and Environment

Incident Investigation

Near misses, incidents and accidents are an important part of a Health and Safety Management System.

By precise identification of root causes and then putting in place effective controls, organizations can avoid reoccurrence to prevent future injury, loss of productivity and asset damage resulting in legislative compliance and reduction in insurance premium.

Job Hazard and Safety Assessment (JSA, JHA, THA)

SRM believes there is no job that cannot be done safely with effective identification of hazards and mitigation strategies.

SRM consultants have proven and diversified experience in identifying the hazards and threats in completing in high hazards jobs.

Our consultants effectively break the job down into a sequence of steps and use questions such as the following, but not limited to these, to identify potential hazards and determine preventive measures to overcome these hazards:

  • Can any body part get caught in or between objects?
  • Do tools, machines, or equipment present any hazards?
  • Are there dusts, fumes, mists, or vapors in the air?
  • Can the worker slip, trip, or fall?
  • Is the worker exposed to extreme heat or cold?
  • Is excessive noise or vibration a problem?
  • Can the worker make harmful contact with moving objects?
  • Is there a danger from falling objects?
  • Can the worker suffer strain from lifting, pushing, or pulling?
  • Is lighting a problem?
  • Can weather conditions affect safety?
  • Is harmful radiation a possibility?
  • Can contact be made with hot, toxic, or caustic products?

Chemicals Risk assessment

Identification of each chemical hazard, the evaluation of their risks and placing of control measures to mitigate these risks secures the health and safety of employees.

SRM follow the following steps in performing the Chemical Risk Assessment:

  1. Identification of the chemicals and their hazards.
  2. Considering who might be affected and how they might be harmed.
  3. Evaluating the risks – what are you doing now and what further precautions are needed?
  4. Documentation and implementation of your findings.
  5. Update and review as required.

SRM provides you with a full assessment and survey carried out at your facility, with recommendations made in an order of priority.

Site Safety Audits

A safety audit is a structured assessment and evaluation of how workplace activities can affect health and safety. It helps organizations in evaluating and optimizing their health and safety programs and improving safety management.

SRM provides the following Safety Audits: 

  1. Regular Compliance Audits: These are carried out on a regular basis in each work area. They cover all working conditions, including hazards, processes, and practices.
  2. Spot Audits: These focus on a specific hazard associated with a specific workstation or work area.

Employee Safety Handbook (HSE Manual)

There is an old saying among OSHA Field Compliance Officers that goes, “If it isn’t in writing, it didn’t happen.” 

 

Our consultants design customized site and organization specific safety handbooks to give employees firm understanding of general questions regarding safety in the workplace.

Permit to Work System

Permit to Work or PTW is a standard operational procedure used by facilities to issue documented permission to personnel to perform tasks that are considered hazardous or non-routine.

It consists of specific instructions based on the nature of the job, and its time and place, along with adequate information of critical procedures to follow.

Our consultants work closely with the site management (generally considering human factors, poorly skilled work force, unconscious and conscious incompetence) to design work permit systems having the following essentials:

 

  1. Sufficient authorizations from assigned personnel throughout the management.
  2. Roles and responsibilities for each task.
  3. Clear identification of hazards that workers may face along with the appropriate precautionary actions to take.
  4. An elaborate set of safety assessment protocols including site audit check, safety equipment check, safety training of personnel check, and more such safety checks which varies according to the type of job.
  5. A record to indicate the status of the work permit – be it closed, cancelled, revoked, re-issued, etc.

Safe Operating Procedures (SOPs)

Safe Operating Procedure (SOP) is a written document that provides step-by-step instructions on how to safely perform a task or activity which involves some risk to health and safety.

 

Our consultants have proven experience in developing the safe operating procedures for critical and high hazardous routine and non-routine activities. We follow the risk-based safety approach in developing the SOPs, clearly mentioning each step with RACI Matrix.

Emergency Response and Incident Command System

A robust emergency plan specifies procedures for handling sudden or unexpected situations to prevent injuries, fatalities and damage to building, stock, equipment, environment and community.

An emergency plan generally includes:

  • All possible emergencies, consequences, required actions, written procedures, and the resources available.
  • Detailed lists of emergency response personnel including their contact details, and their duties and responsibilities.
  • Floor plans.
  • Large scale maps showing evacuation routes and service conduits
